OUR PROCESS
Initial Consultation
After referral, we conduct initial interviews, document and data reviews, observations, and engagement.
Behaviour Plan Development
We begin with a Quality of Life assessment, evaluating strengths, risks, and goals, as well as identifying any existing restrictive practices.
Implementation
We provide training and coaching for key support people and family. We listen to ongoing feedback, review data, and update accordingly.
Outcomes and recommendations
After each service, we provide a letter of recommendation to assist in obtaining the appropriate level of support.

Is Momentum 360 registered with the NDIS?
Yes, Momentum is a registered NDIS provider, dedicated to delivering the highest standards of compliance and quality for NDIS participants as well as other clients.
Do I need to be a NDIS participant?
No, you do not need to be an NDIS participant. Momentum provides support to a range of individuals and organisations, including schools, government departments (such as Education, Corrections, Families and Fairness, and Communities), as well as private clients like parents seeking support and guidance. We offer case consultations, supervision, and tailored services to meet diverse needs in PBS.
How do I make a referral?

What locations does Momentum 360 service?
Momentum provides face-to-face and hybrid support across Australia, with practitioners in WA, SA, and Victoria. Our central hub, “Momentum Central,” is located on the border of Northeast Victoria and Southern NSW, serving as the core of our operations with multiple practitioners in this area. Our main clientele is currently based in Shepparton, Albury, Echuca, Apollo Bay, Melbourne Northern Suburbs, Adelaide, Perth, Geraldton, Kalbarri, and Broome. We are continuously expanding and offer exciting opportunities to all Australians, wherever they live.
What is Positive Behaviour Support?
Positive Behaviour Support (PBS) is an approach to supporting people to live a fulfilling life and minimise the need for challenging behaviours. PBS provides a framework for learning about a person and their support needs, and working with them to build the supports and systems that will help them live a good life.
PBS is grounded in person-centred practice and seeks to protect a person’s human rights. It is focussed on systems change and skill development (for the person and support networks) and utilises behaviour science and other evidence-based approaches (therapies, techniques, strategies) to support quality of life improvements and address challenging behaviours.
How does PBS Help?
PBS provides an approach to understanding the environment and systems around a person and to identifying what changes, approaches and supports will help. Systems may include a person’s home, community, school, workplace, etc. – all environments that are important for a person.
A PBS practitioner will start with identifying the person’s values (what’s important to them; how they want to live their life) and will work closely with the person to ensure PBS processes align with what is important to them (e.g., who will be involved and how they want to be supported).
The practitioner will develop support and intervention strategies that help a person achieve their goals.
Do you have a plan writing model?
Multi-element Behaviour Support Plans are a vital aspect of providing tailored, person-centred, and strength-based support for individuals with disabilities. These plans integrate diverse strategies and interventions that reflect the unique needs and preferences of each participant, ensuring their voices are central to the process.
Do you offer Restrictive Practices?
This support is offered when participants exhibit challenging behaviours or when restrictive practices are employed in their care. The aim is to reduce or eliminate such practices, thereby promoting the well-being and rights of participants.
